HOW TO GET STARTED
4 SIMPLE STEPS
Step 1
Verify patient eligibility by filling out our HIPAA compliant teleplacement referral form
Step 2
Virtare will validate insurance coverage and schedule a practitioner appointment
Step 3
Nurse Practitioner comes to your SNF to complete necessary documents and procedures
Step 4
Within 72 hours, the facility has a patient ready for a safe discharge
WHAT IS COVERED?
Our services are billed through the patient's insurance plan. There is no cost to you and there is little to no cost to the patient if they are a part of one of the following plans.
IN NETWORK RESIDENTS
1. United Healthcare
2. Aetna
3. Tricare/VA
4. Medicare Fee For Service (Traditional Medicare)
5. Anthem Blue Cross Blue Shield
6. Health Net (Coming 2022)
7. Centene (Coming 2022)
8. WellCare (Coming 2022)
OUT OF NETWORK RESIDENTS
If the prospective patient does not have one of the aforementioned insurance plans, Virtare will do all of the discharge paperwork for a nominal fee of $250 billed to the facility
